FOIL Request Form

Fundraising Request Forms must be submitted to the Building Principal or the Athletic Director at least 30 days prior to the event and MUST receive approval before any fundraising activity begins. This will allow sufficient time for requests to be reviewed and approved before the selling and advertising period begins.

Before completing the request form, please review the 5521AP Fundraising Procedure and the Fundraising Event Chart below to ensure your proposed fundraiser does not duplicate, overlap, or compete against approved fundraising events. 

Completing the Fundraising Request Fillable Form

Important: Download and save the PDF to your computer before completing it. The completed form must be printed and signed before submission.
